This informative title provides nurses with specific, practical advice on documenting a wide range of situations from caring for a patient with a myocardial infarction to witnessing a patient sign his will. In clear, concise language, the book gives detailed explanations of how, what, and when to document in nearly 100 of the most common, most important situations nurses face in practice. Each entry tells exactly what to consider and what to document so that the nurse can ensure quality patient care, continuity of care, and legal protection for the nurse and the institution.
Found this to be very helpful and insightful. Would highly recomend for staff education.
"Surefire Documentation" Review

This is a wonderful book. It was given to me to review by one of the charge nurses where I work--a long term care facility in a small town in Iowa. This text not only gives very good instruction on documentation, it is organized well. Each cover includes a quick reference guide to assist with finding specific areas of concern. It is well written and includes text boxes with more specifics and concrete examples. I have recommended it to my Director of Nursing Services as a resource that could be used in the other 2 nursing facilities in the corporation. An excellent reference text with easy to find entries! I would highly recommend it to anyone who wants a ready reference.
