This is a great guide for field paramedics. It has thorough and accurate information about everything from home meds to the meds you have on your bus, including contraindications, emergency dosages and side effects for those occassional moments when you draw a blank (like when you're in a geriatric home where the thermostat is turned up to 150.) The only downfall, is it's a little tricky to look up drugs by their trade name, but as long as you mark the index with a post-it note or something, it will work fine. Also, there is plenty of room for notes. In addition to its ideal size, it has a little tougher cover than the Pharmacopeia so it lasts a little longer in your back or cargo pockets.

Not exhaustive, but very handy. Fits in your pocket. Good to refresh your memory on the job. You can always look up details in a larger book if needed. I am a clinical instructor. Fellow instructors & students who see my NPDG want one for themselves.
